2025-2026 LFCDS Medical Forms

Each year, families are required by the state of Illinois to turn in medical forms for each student enrolled.
Please carefully read the information outlined below to determine which categories pertain to your child(ren).

All required medical forms are due no later than August 1, 2025

This information is required for all students.

The Medical Update Form and the Emergency Contact Form will be completed online through the Portal only and they need to be updated annually.  Families with students in Grades 5 - 8 will also have a Concussion Information Acknowledgement to complete as well.  Please click HERE for instructions on how to complete these forms on the Portal or refer to the video tutorials below.

This information is required for
specific grade levels. 

Please download and complete each form that pertains to your child(ren) and return them to the school nurse.

New Student to LFCDS
Certificate of Health Examination
Eye Examination (SK-Grade 8 students who are new to IL. Students moving from a school within IL do not need to submit an eye exam.)

This information is required for students
with specific medical needs and needs to be updated annually. 
 
Please download and complete each form that pertains to your child(ren) and return them to the school nurse.

Required to administer any type of medication during school.

Allergy & Anaphylaxis Action Plan 
Required if your child needs possible use of EpiPen.
 
Recommended if your child has an inhaler.
 
Required if your child has diabetes. 




Please be sure to schedule any doctor/dental/eye appointments early to ensure the prompt return of the health forms.
 
These medical requirements are mandated by state law and are intended to ensure your child’s health and well-being. Any student whose forms are not on file may be denied admission to class and school-related activities.
